Registration Requirements
You are ready to begin the process of enrolling your child when you have met the following requirements:
Registration Documents
Please download and print ALL of the documents below. Bring the completed packet to school when registering your child. You will need Adobe Acrobat Reader to view these documents. These forms are also available in the school office for your convenience.
Please bring the following items to school for registration of returning and new students

if dependent is not listed on the orders, verification of sponsorship from HRO must be provided. |
Returning Students
All returning students must present verification of current immunizations. You may bring DoD immunization verification form DSJN 122b which may be obtained at the immunization clinic, Yellow Shot Card, or Copies of child's medical record for verification of immunizations.
As of SY 2006-2007, ALL children must have:
* DPT - a minimum of 3 doses with the last being administered after the child's 4th birthday.
* OPV or IPV - a minimum of 3 doses with the last being administered after the child's 4th birthday.
* MMR - a minimum of 2 doses. The last doses is recommended to be given after the child's 4th birthday.
* Hepatitis A Series - 2 out of 2 shots (or currently in progress)
* Hepatitis B Series- 3 out of 3 shots (or currently in progress)
* PPD - is recommended not required
* VARIVAX - All children must have vaccine if they have NOT had a reliable history of chicken pox.
* Hib (Haemophilus influenza type b) series - At least one dose for children 3-4
NOTE: Any students without verification of all immunizations will not be allowed to register or attend school until appropriate documentation is provided.
New Students
New students must also submit the following documents:
* Student's Passport or a copy of Birth Certificate
* Student's Social Security Number
* Completed Registration Packet: - This is available above or you may pick one up at the schools registrar's office.
NOTE: In order to ensure a smooth transition for new students into The Sullivans School, teachers need time to prepare all necessary materials. With this in mind, the following policy is in place at our school:
When a parent registers a new student prior to 11:00am, the student is invited to join his new class the next school day.
When a parent registers a new student after 11:00am, the student is invited to join his new class two days later.
For example, if a student is registered at 9:00am on a Tuesday, that child will join his new class on Wednesday. If a student is registered at 1:00pm on Tuesday, that child will join his new class on Thursday.
